PhysioCare Rehabilitation participates with most major insurance plans. We will gladly answer any questions you may have regarding your insurance plan, authorizations, or any other related questions.

We are a certified and participating Medicare provider and we accept Workers' Compensation and Motor Vehicle (PIP) Insurance. Our staff is trained to assist you.

An insurance referral and prescription from your physician is generally required for insurance reimbursement, however, we also welcome private pay patients. Referrals should be made out to the proper facility. For more information, please do not hesitate to contact us.

As a courtesy to our patients, we submit claims to all insurance companies. To ensure your claim is submitted in a timely fashion, please make sure to complete the Patient Information Form.

Auto Accident and Workers Compensation

We will file Motor Vehicle claims and Workers’ Compensation claims. Please fill out the Patient Information Form to ensure that we have all the information required to file a clean claim.

Self-Pay

Payment at time of service is required. If your services are not covered by any insurance, we do offer a prompt payment discount for payment at the time of your visit. Forms of payment accepted include Visa, MasterCard, check and cash. In cases of financial hardship, payment plans may be available.
