What is Physical Therapy?
Physical therapy (also known as physiotherapy) is the treatment of physical dysfunction or injury by the use of therapeutic exercise and the application of modalities that are intended to restore or facilitate normal function or development. You may feel discomfort during the initial phase of therapy. It is important to complete your entire physical therapy program in order to eliminate discomfort. The benefits of physical therapy include achieving pain-free optimal functioning, enhancing the quality of life, improving strength, mobility and stability.

Prior to your first appointment, please be sure to bring these documents with you:
1) Your doctor's prescription
2) Any pertinent X-Ray, MRI or CATscan reports related to your condition with you for your first visit. Please obtain these records prior to your appointment.
3) Your insurance information
- If private health insurance, your referral from your treating physician
- If related to a motor vehicle accident (PIP insurance), the name of your insurance company, claim no., and adjuster's phone number.
- If workers' compensation bring the name of your insurance company, claim no., and adjuster's phone number. |
4) PhysioCare Rehabilitation initial documents. Please plan on arriving 15 minutes prior to your appointment time to complete the necessary paperwork. The inital paperwork is available for you to download by clicking HERE. You can save time by completing them at home. Once this document is opened, please fill in the information on your computer screen, print it,
and then bring it to the office for your initial consultation.

5) If your rehabilitation is regarding your lower body or back, please wear or bring shorts to your appointment. Also consider bringing athletic shoes if pertinent to your condition.
6) If your rehabilitation is regarding your upper body or back, please wear or bring a tank top, halter top or other appropriate clothing which reveal the body part being treated.
7) If you are not sure of the date and time of your appointment, call our office first at 856-435-2323. If it is necessary to cancel an appointment, call the office as soon as possible to reschedule.
